What happens in my FFS consultation?

During your face-to-face FFS consultation, the surgeon will discuss your case at length and address any questions and concerns in detail. We offer our honest opinion from a clinical and aesthetic perspective. However, the patient’s requirements and objectives are also a very important consideration.

Your health and realistic goals are our main concern when carefully considering your case. An exhaustive evaluation is necessary in order to build a personalized treatment plan for FFS surgery that will meet your needs. This may mean several consultations via email, Skype and finally meeting in person to discover the best plan for you. Therefore, it is our policy not to publish “cookie-cutter style” facial feminization cost lists, as each case, and face, really is unique.

How can I book different types of consultations?

Fill in the form on our webpage with the required details (*) to request the type of consultation preferred. The types of consultations available are:

Where and when can I have a consultation with the FFS surgeon?

During your 30-40 minute meeting with the FFS surgeon, the primary aim will be to clarify your expectations and questions regarding facial feminization. If you choose this type of evaluation on the form, additional fields will appear to attach photos so that you may describe, for example, what facial features you wish to change. Eventually, we will need to know your Skype user ID to connect with you on the day of the appointment.

What photos should I send for the Skype/email evaluation?

Please read the instructions below regarding the photos and details we require in advance. We cannot provide an appointment or evaluation without the required information and photos for an appropriate analysis.

  1. Describe the features you are uncomfortable with, if possible. It is also important to list any previous facial treatments, surgeries or injuries.
  2. 3 photos with your hair pulled back (frontal, profile, and lateral 45 degree angle, as in the examples below).

Please note the following tips:

  • Don’t hold the camera yourself and try not to use mirrors.
  • Keep the camera at least 6 feet (2 meters) away from your face.
  • Camera angle: the camera should be at eye level. Try to keep your head and gaze horizontal.
  • The pictures should be well lit and in focus, but not in direct sunlight.
  • Keep a blank expression, without a smile.
  • It’s best to turn your whole body to get the different angles rather than just turning your head.
  • Keep your hair tied back so your cheeks and forehead are revealed.
  • Picture size: up to 1 MB is sufficient.
  • Try to have a blank and neutral color background.
  • Don’t wear glasses.
  • It’s fine to wear light make-up, but avoid heavy eyeliner or eye shadow.

How much does the consultation cost?

FFS consultations are free as part of our policy to eliminate barriers to accurate information on facial feminization surgery.

Our budgets are personalized according to the individual needs of each patient, since each FFS treatment depends on an individual’s needs and objectives. For this reason, we do not publish a list of prices although facial feminization costs usually run between 8,000 and 25,000 euros, depending on the treatment and duration of the surgery.

May I bring someone with me to my appointment?

Yes. We actually encourage you to bring your spouse, significant other, or a friend along with you to your appointment.

How long will my appointment last?

Most in-person appointments in Marbella last one hour or possibly longer depending on the complexity of the case. Videoconferences and consultations held abroad usually last around 30-40 minutes.

What should I bring with me to my appointment?

If you have dental or cranial X-rays, it would be very helpful to bring them or send them in advance. For digital simulations with the facial recognition specialist, Alexandra Hamer, you should come with neutral, light makeup. Just bring a list of questions so you don’t forget anything. There is no pressure to book and the consultation is completely free of charge, whether it is a first, fifth or checkup consultation.

How do I get to the FACIALTEAM offices in Marbella? Malaga Airport and the Marbella area

Malaga International airport (AGP) is only 40-60 minutes from Marbella. More than 60 airlines have direct flights. From there, we will take care of your transportation to your hotel and to the hospital.

Will I have problems communicating in Marbella?

About 40% of the population in Marbella, a popular tourist destination and ex-pat colony for northern Europeans, is not Spanish. For that reason, you will often find people who speak English and/or other languages. However, there will be times when you will encounter non-English speakers, so please be patient!

Where can I stay while I’m in Marbella? Accommodations

Aldea La Quinta is our first recommendation, but a list of other sites is also provided. Prices vary depending on the season, the highest being from June to August. There are a number of other options available for every patient’s budget, ranging from luxurious 4-star hotels with views of the sea or mountains to apartments equipped with kitchenettes. The costs of housing, meals and international/national travel are the responsibility of the patient (while not in the hospital), unless otherwise indicated.

We will provide a detailed schedule in advance, so you know exactly where you should be and when. All local transportation for appointments is provided.

Smoking

As smoking counteracts the wound healing process, we strongly recommend not smoking for 3 weeks before and 3 weeks after the surgery.

Health

Please notify us of any health conditions you may have and any medications you take, which will be kept confidential. HRT does not need to be suspended before surgery since we will administer an antithrombotic agent to counteract the clotting effects of the hormones.

Swelling and bruising

We use a special cooling mask placed immediately after surgery and kept in place almost constantly for the first 48 hours. This mask maintains the surface tissues at the optimum temperature to reduce swelling and bruising and alleviates discomfort resulting from the operation.

Once back at the hotel or at Aldea La Quinta, follow our directions for cooling to alleviate any swelling. Use the compression bandages as directed for this purpose. Bruising and swelling will peak 2-3 days after the surgery and will subside about 7-10 days later. Some swelling may be evident for several weeks after surgery, especially when bone reconstruction surgery is involved. However, any residual swelling that lasts for more than a month is not noticeable to others. During this time, we recommend that the patient take it as easy as possible and avoid overexertion.

Emotions

During the first days following surgery you are swollen, bruised, and sore, both physically and mentally. We are here to give you all the support and care you need. Additionally, a professional psychological support service is available. We also recommend preoperative treatment to assist with the transition.

Lumps and scabs

Tightness, hardness and lumpiness around the face, jaw, and neck is common and will subside over time. After 24 hours, manual lymphatic drainage is administered to aid tissue recovery. Do not pick or remove any scabs or crusting.

Pain

The pain is surprisingly moderate with facial feminization. Most patients will experience discomfort more than pain, which is usually easily controlled with normal painkillers. However, if you continue to experience pain, you should contact us. Pain medication is generally not needed after one week.

Numbness

A total or partial lack of feeling near the treated area is normal and will gradually improve with time, usually between 3-6 months. In some cases, this may take up to one year.

Bleeding

You may experience light bleeding in or around the incision areas, which is not a problem. However, if you find any large and/or hard collections of blood or experience excessive bleeding, please alert our office as soon as possible.

Medications

We will provide you with a postoperative medication packet with antibiotics and painkillers, modified according to each patient’s needs. Other complementary medicines or ointments will be recommended as needed. We suggest you consult your physician before resuming any discontinued medications.

Cleaning external incisions

If you have sterile strips covering the wound or a bandage, do not touch them and keep them dry. To clean your face, use a gentle soap. With clean hands, simply apply it to your face, avoiding the incisions. Rinse your face as normal with cool water and carefully dab it dry with a clean cloth. Allow it to air dry completely or use a hairdryer on a cool setting. Any remaining moisture in the wounds should be removed by dabbing them with gauze and a small amount of 60% alcohol. When thoroughly dry, you may opt to use an antibacterial ointment to keep the incisions clean and soft, which helps them heal quickly. Never pull the skin and be gentle! Keeping the incision areas as clean as possible will also help us remove your sutures more easily. If necessary, we will see you for wound care in the clinic.

Personal hygiene

You may shower 24 hours after your surgery. Baths are acceptable, if they are short and not too hot, but avoid soaking any of the treated areas in water. Your incisions can get wet but should not be soaked for long periods. We recommend using Betadine shampoo. Do not bend over to wash your hair; instead, tilt your head back in the shower. When washing your hair, always gently massage towards your incisions, never away. Do not be alarmed if the wound bleeds a little while washing your hair. Be gentle with your face and scalp, especially when toweling dry and do not apply pressure to the scalp. Only use a hair dryer on the low, cool setting for the first 8 weeks after the surgery. If you are candidate for a Simultaneous Hair Transplant (SHT), please follow the specific postoperative instructions.

Movement and food

At the hospital, once the catheter is removed (if it is used), you may try to get up gently and eventually take short slow walks in the hallway, unless your doctor says otherwise. During the first week after surgery, you should rest as much as possible. However, it is good to take short, low-impact daily walks once you feel better. You also should avoid bending over at the waist for the first six weeks after surgery to minimize the amount of blood flowing to the area. Instead, bend at the knees and/or turn your entire body if necessary. Keep your head elevated at all times. Avoid excessive chewing by consuming soft foods like fruit shakes, yogurt, soup, eggs, pasta, rice, or pudding following surgery. With forehead reconstructions, you should not blow your nose for 2 weeks and sneezing should be done with your mouth open.

Eating

It is not advisable to eat solid food during the 8 hours before surgery. The last meal before fasting should be light to ensure an empty stomach. It is best to limit your intake to soft foods at room temperature or cooler. Do not drink liquids during the 8 hours prior to surgery. Soft drinks, tea, coffee, and sodas, which may delay clotting or wound healing, should also be avoided the day before.

Sleeping

When sleeping, use two pillows to prop up your head and support your neck, trying to keep your head slightly higher than your heart. Do not sleep on your side for one week and if necessary, place pillows on the sides of your face to keep yourself from turning your face while sleeping.

Work and play

We highly recommend total rest and relaxation for two full weeks following surgery. If you give your body ample time to recover and relax physically, emotionally and mentally, you’ll heal faster in the long run. A positive mentality has also been proven to aid healing, so practice your new smile!

Physical activity that raises your blood pressure or produces sweating should be resumed carefully and slowly after 6 weeks. Travelling is permitted after one week, but ask for help with any heavy luggage. After two weeks, you may resume high-demand driving, sedentary work, and light exercise like slow walking. Intense exercise (running, Pilates) may be resumed 6 weeks after your surgery, depending on your recovery. Yoga, especially upside-down poses, should be avoided for 8-10 weeks. Ask us if you plan to do any high-impact or heavy contact activity soon.

Beauty

Do not use hot hair rollers or get your hair cut, permed or colored for six weeks. You can apply light makeup gently after 1 week. Take care with heavy ear jewelry. Strong anti-ageing skin treatments should be avoided for 3 months and electrolysis can be restarted 6 weeks after surgery.

Sun

Avoid sun exposure to the surgical area for six weeks and use sunscreen, even on cloudy days, on the incisions for at least one year. This will improve the quality of the scar and help it fade over time. Sun exposure causes skin damage, which leads to early signs of aging.

Support

Our patient coordinators will take care of you from the first contact to long after your surgery. They’ll help you with logistical details such as accommodation and organizing appointments as well as airport transfers. They’ll also give you a mobile phone so you can reach them at the touch of a button, 24 hours a day.

Should I bring someone to my surgery?

Yes, it is preferable to come with a close relative or friend during your stay in Marbella.

How long will I be in the hospital?

Most patients must spend one to two nights in the hospital after their surgery. After this, Aldea La Quinta is our first recommendation during the healing process (around 7 days).

How do I reach the doctor after hours if there is an emergency?

During your postsurgical hospital stay (24-48 hours), the patient care nursing team will be in contact with your patient coordinator and one of the surgeons on your team. If there is an emergency (severe problems breathing, excess bleeding, heart palpitations, etc.) while you are staying at Aldea La Quinta or another housing option, you can contact the patient coordinator, HC Marbella International Hospital or the emergency number (112) at any time.

How soon after facial feminization surgery can I travel?

There are usually a number of follow-up appointments after your surgery you need to complete before leaving town. We usually recommend not travelling for at least 8-10 days after your FFS.

What happens if I need to see the doctor when I get back to my country?

First, contact us during our office hours or after office hours through HC Marbella International Hospital. We collaborate with several doctors in different countries who can help you. If we do not know a doctor in your area, we advise you to consult your personal physician. After your visit, we will discuss the problem with your physician and recommend the appropriate treatment. In extremely rare cases, you would have to come to Marbella.

Can I dye my hair before the forehead surgery?

No, you cannot dye your hair during the 14 days prior to the surgery, since the dye can irritate the skin and facilitate the spread of germs. You should also not use hair masks, lotions, powders, etc. during the 24 hours before your surgery. These products can interfere with the solution used to prepare your skin before the operation.

When can I pluck my eyebrows after the surgery?

Generally your forehead is swollen for a while (months) after forehead surgery. The brows will settle down as you heal from the surgery. For this reason, you should wait about 6 weeks to 2 months before having your eyebrows shaped, because pulling the soft tissue in the area can interrupt the healing process and the tissue adaptation to the bone.

When can I wash my hair after the surgery?

You can wash your hair 24 hours after surgery. You should clean the coronal incision with Betadine shampoo, always gently massaging towards your incisions, never away from them. You should also keep the area clean and dry. Wash the area with water and Betadine shampoo once a day and then dry it with a hairdryer on the cold setting.

When can I use hair conditioner after the surgery?

It is fine to use hair conditioners as long as they do not contain components that irritate the scalp.

When can I color (dye) my hair after the surgery?

We do not recommend dyeing your hair until 6 weeks after surgery, after which time the scar is completely closed and there is no danger of the dye entering and irritating the incision area.

Can I blow my nose?

You should not blow your nose for 2 weeks, since the nose is connected to the frontal sinus and exerting pressure could produce an emphysema (the accumulation of air in the soft tissues) in the forehead and periorbital areas. If an emphysema does develop, do not be alarmed; the air will reabsorb over time.

Can I sneeze?

Yes, but always with your mouth open to avoid performing a Valsava maneuver and producing an emphysema.

Is it normal to experience some dripping blood after forehead surgery?

Yes, some amount of dripping blood after surgery is normal since the area worked on, the frontal sinus, is connected to the nose.

Will my hair fall out after a hair transplant?

Shock loss. This is not common, but it can be a collateral effect of a hair operation and may occur in the both the transplant area and donor area. If you have any questions about this, contact us.

When can I wash my hair after the hair transplant?

It is important to follow these instructions:

  • For the first 7 days, you should soak the transplant area every two hours with saline solution to keep it moist. Apply the saline solution by spraying it with the dispenser, without directly touching the recipient area. If this is not done, a scab may develop that will stop the hair from growing. If you run out of the solution, you can fill the dispenser with physiological saline or consult your coordinator if you prefer.
  • From days 2 to 7, spray Betadine shampoo directly on the transplant area. Apply it gently, without massaging or touching the recipient area. When washing the area, the water pressure should be very low and you can use a small cup of warm water to rinse the shampoo. Do this once a day. You can also apply Betadine shampoo to the rest of your scalp.
  • From days 8 to 14, continue applying Betadine shampoo to the transplant area, beginning to softly massage your scalp in circles using the tips of your fingers. At this time you can use normal water pressure when rinsing. If you wish, you can also use a gentle shampoo like Pilopeptan shampoo after using the Betadine shampoo. Continue to wash your hair 1 or 2 times a day.
  • After day 14, if you have correctly followed the postoperative instructions, you should not have any scabbing. If you do, you must wash the area twice a day. You can apply a small amount of olive oil to the transplant area one hour before washing your hair to soften the area.
  • After day 15, you can use a gentle baby shampoo or a gentle normal shampoo to wash your hair and the recipient area. At this point you can return to your normal hair washing routine.

Correctly following these instructions will guarantee good results for your hair transplant.

What is the best position to sleep in?

You should sleep on your back. Place your head and upper part of your body at a 45º angle, using pillows to support you. This will help reduce swelling. You should maintain this position for the first week after the operation.

How quickly will the inflammation disappear?

This is a postoperative side effect of hair surgery and will disappear in the following 4-5 days. Your resting position, the use of an ice pack on your forehead and refraining from taking long walks will help reduce the effect.

Can I sunbathe after a hair transplant?

Cover the recipient area and avoid direct strong sunlight for the first 6 weeks. You should take care not to burn your scalp during the first 6 months.

When can I do physical activity after a hair transplant?

For 6-8 weeks, you should avoid lifting heavy objects or making brusque movements that could exert excessive tension on the donor area. You can begin to take short, gentle walks after the first month and run more intensely after 2 months.

When can I swim in the sea/swimming pool?

You must wait a minimum of 1 month before swimming in the sea (remember to always protect yourself from the sun). You must wait 3 months before swimming in a pool because of the chlorine.

Is it normal to experience some loss of blood or infection after a hair transplant?

Some loss of blood is inevitable during and immediately after the operation, although it is not common later. If bleeding continues, apply pressure to the point with a clean sponge for 20 minutes. If it still persists, contact FACIALTEAM. On very rare occasions, the transplant area may become infected, irritated or swollen. Take care not to bump your head and use only clean products.

Will I feel some lack of sensitivity?

Some numbness may occur as a result of the small nerve endings being cut or stretched during the operation. You should not be concerned as this is temporary. In some cases, complete recovery may take several months, as long as the scarring process lasts.

When can I color (dye) my hair after the hair transplant?

You should not dye your hair until 6 weeks after the hair transplant.

Will my hair fall out after a surgical hairline lowering?

As the incision is just inside the hairline and the scalp is moved at the deep hair root layer, you should not lose much hair. Any hair loss is, generally speaking, mild and temporary.

When can I wash my hair after surgical hairline lowering?

You may wash your hair 24 hours following surgery for brief periods of time using warm, not hot, water. Your incisions can get wet but should not be soaked for a long time. We recommend using Betadine shampoo. Do not bend over to wash your hair; instead, tilt your head back in the shower. When washing your hair, always gently massage towards the incisions, never away from them. Do not be alarmed if the wound bleeds a little while washing your hair. Be gentle with your face and scalp, especially when towel drying, and do not apply pressure to the scalp. Use a hair dryer on the low, cool setting until 8 weeks after surgery.

Can I wear makeup after my rhinoplasty?

You can apply your usual makeup without any problems.

Can I wear my glasses after my rhinoplasty?

For at least the first two weeks, you should wear contact lenses, because the frames may be uncomfortable and leave marks that delay the final result while your nose is still swollen.

How quickly will the inflammation disappear?

The healing process after a rhinoplasty varies from person to person, depending on the skin type and the specific procedure performed. In general, maximum swelling occurs within 24-48 hours after surgery, after which it gradually subsides until it completely disappears around 6 months after surgery, although it can take up to one year. Between the first and second week you will notice a significant reduction in the inflammation and 2 months after surgery, between 80-85% of the swelling will be gone, at which point you can make a preliminary assessment of the final result.

What should I do after my rhinoplasty?

It is important to follow these instructions:

  1. Take the medication prescribed by the doctor after meals.
  2. Do not smoke, which increases the risk of infection and impedes healing.
  3. Sleep with your head elevated (using 2-3 pillows) for the first week.
  4. Avoid direct sunlight and saunas for 6-8 weeks after the surgery. The sun and extreme heat increase edemas (swelling), slowing the healing process. Use sunscreen, even on cloudy days, for at least one year.
  5. Avoid crowds and sports where there is any risk of your face being hit for at least 6 weeks.
  6. Avoid wetting the nose bandage (patch) during the first week. You should always wash your hair with your head tilted back to avoid direct contact between your nose and the water.
  7. You can wash your face carefully. You should apply Thrombocid Forte 2-3 times a day to any bruised or inflamed areas until they recover. You can apply makeup one week after the surgery.
  8. Apply Vaseline (petroleum jelly) in the area with the stitches.
  9. Clean inside the nostrils with a sterile saline solution spray.

What is normal to expect after the stitches are removed

You may note a “falling” sensation after the sutures have been removed. This is a common sensation, but the lip is in fact supported by internal stitches and will not return to its original position. The relatively tiny scar may initially have some openings, which should close after a few days and are no reason for concern.

Can I have laser hair removal done before a lip lift?

No, you should not have a laser hair removal session for at least 14 days before your surgery, since the skin can become irritated and allow germs to enter more easily. If the patient’s skin is particularly sensitive, this procedure should not be done for 4 weeks before the surgery. Do not shave on the day of the surgery, which may cause small abrasions or nicks in the skin and allow germs to enter. Shave the night before the surgery.

When can I begin laser hair removal or electrolysis after my lip lift?

You can start laser treatment or electrolysis 6-8 weeks after the surgery, when the inflammation in the area has decreased and the scar is more mature.

When can I wear makeup after my lip lift?

You can gently apply light makeup after 1 week. Strong anti- ageing skin treatments should be avoided for 3 months.

How should I take care of my lip lift?

To improve circulation in the lip lift incision, we recommend gently massaging Rosa Mosqueta oil one week after the stitches are removed, after which you should use a silicone gel (Dermatix) for at least 3 months and up to one year, beginning two weeks after the stitches are removed.

Is anesthesia necessary? Is the procedure painful?

The procedure is not painful, although at times the use of a local anesthetic is necessary prior to the procedure if it is done in isolation.

How long does the procedure last?

The procedure takes from 15 to 45 minutes.

How many sessions will I need?

Filling sessions using hyaluronic acid are usually done approximately every 6 months, since this material is reabsorbed. It can be done at any time of the year. Which areas will appear swollen or bruised after facial feminization surgery?

Immediately after surgery the swelling will appear mainly in the soft tissues (cheeks, lips, around the eyes). During the first few days, the swelling will go down to the jaw and neck, due to the effect of gravity. However, swelling and bruising differ from person to person and can occur anywhere.

We utilize a special cooling mask (hilotherapy), which is placed immediately after surgery and kept in place almost constantly for the first 24-48 hours. This mask keeps the surface tissues at the optimum temperature to reduce swelling and bruising and alleviate discomfort due to the operation.

Once back at the hotel or at Aldea La Quinta, follow our directions for cooling to alleviate any swelling. Use the compression bandages as directed for this purpose as well. Bruising and swelling will peak 2-3 days after the surgery and will subside about 7-10 days later. Some swelling may be evident for several weeks after surgery, especially when bone reconstruction surgery is involved. However, any residual swelling that lasts more than a month is not noticeable to others. During this time, we recommend that the patient take it as easy as possible and avoid overexertion.

Does manual lymphatic drainage have any contraindications?

Lymphatic drainage cannot be administered if the patient has signs of tumor diseases or an infection.

This treatment begins the day after the surgery with a session in the hospital room. The treatment is administered comfortably, with the patient on the hospital bed. Manual lymphatic draining procedures are done extremely carefully (the pressure is very light) and are always pleasant for the patient.

How many manual lymphatic drainage sessions are necessary?

The patient will receive 2 MLD sessions. We recommend continuing the treatment for two more weeks, spacing the sessions out according to the recovery.

Who should administer manual lymphatic drainage?

The administration technique must be precise and the movement of the skin during the first week must be minimal so that it retracts correctly. Done incorrectly, the procedure can damage the lymphatic vessels and the bonds between the skin and the lower structures. Additionally, moving the skin too soon can impair good lymphatic regeneration and impede the creation of the bonds that connect the skin with the underlying structures. Excessive movement can result in flaccid skin, negatively affecting the results of the operation. For these reasons, it is very important that the massage be done by a professional physiotherapist specialized in post-surgical manual lymphatic drainage.

As its name indicates, manual lymphatic drainage is done using the therapist’s hands, with no equipment or machines. Although some devices can also accelerate lymph circulation, they should be used as a complement to MLD, not instead of MLD.

I’m afraid to have a facial massage after the FFS. Is manual lymphatic drainage aggressive?

Manual lymphatic drainage is a gentle technique that uses pleasant, surface-level actions. It is not aggressive and the kneading and intense manipulation characteristic of some massages are not part of manual lymphatic drainage. You should not subject your tissue to any rough lymphatic drainage or massage after your surgery.

How long will it take for my hematomas to disappear?

Hematomas usually disappear from 7-10 days after surgery, although this depends on the patient. We recommend the use of Thrombocid or a cream containing arnica to help eliminate them.

Are the different acupuncture techniques painful?

Acupuncture uses very fine-gauge, sterile, single-use needles and when the needles are inserted correctly, the process is barely noticeable.  There may be some discomfort in certain areas, but generally speaking acupuncture is practically painless. When electrostimulation is applied to the needles, the resulting tingling sensation is completely tolerable, thanks to the very low intensity. The laser used is a very low-power instrument and is not painful. Depending on the particular area being treated, you may notice some local heat, but it is not uncomfortable, and the procedure notably reduces inflammation and edema. With auriculotherapy, the type of puncture is more painful than with conventional acupuncture needles, but most patients are usually able to tolerate it quite well.

Does acupuncture have any adverse effects

Acupuncture is a very safe technique with almost no adverse effects.

Does auriculotherapy have any contraindications?

It is contraindicated for patients taking anticoagulant or antiaggregant drugs and patients with coagulation alterations.

Does electroacupuncture have any contraindications?
