Simultaneous hair transplant surgery (SHT) has led to a revolution in facial feminization at Facialteam.

Why is the hairline important in gender identity?

The forehead and orbital area (fronto naso orbital complex) is one of the most revealing gender features. The frame of the eyes is decisive in the perception of a masculine or feminine appearance in varying degrees. 

Like the hairline, the male hairline typically has an “M” format, with recessed entries. However, a feminine hairline is characterized by being more rounded without the typical male-pattern balding.

This surgery is performed in the same operating room and with the participation of more than 10 professionals, including surgeons, nurses, technicians and assistants. It is one of the most important for our patients since the change achieved has a very important role in the feminization process.

Key steps in a simultaneous hair transplant surgery

What does a simultaneous hair transplant consist of? In SHT surgeries, a frontoplasty (reconstruction of the forehead) is performed and, in turn, the surplus hairs are employed for grafting in the donor sites along the forehead’s hairline. 

What we achieve is that the entire hairline, forehead and orbital area are simultaneously transformed into a more feminine format in a single intervention, thanks to the collaboration of both maxillofacial and hair transplant specialists. 

What’s the step by step process of this type of surgery from the point of view of the hair surgery team?

1. Analysis and Diagnosis

First, we begin with an analysis and diagnosis of the patient. How many follicles will ideally be required for the size of the grafting area? Also, the appropriate donor site for the access incision is identified to ensure an optimal number and quality of hair follicles. Does she suffer from any type of hair pathology? If so, it will be essential to define it in advance to ensure that the hair transplant can be carried out.   

2. Define the right Treatment

We will define the treatment if necessary. In the case of androgenic alopecia, medical treatment before and after surgery will be essential since it is important that the transplant harmonizes with the rest of the hair in the recipient area and that the alopecia does not progress.

3. During the Intervention

Already immersed in the surgery, the team of technicians proceeds to separate follicle by follicle from the excess skin area of the frontoplasty, for its subsequent implantation in the hairline; going from having an M shape with typically masculine entries, to a more rounded and feminine shape.

One of the factors that we take into account the most when it comes to achieving a feminine and natural aesthetic is looking for a high density of follicular units (FU) per cm2 (FU / cm2). Therefore, the result is more satisfactory for the patients.
