The facial feminisation procedures I had in November 2011 were: forehead/orbital reduction and eyebrow lift via coronal incision; rhinoplasty, and submandibular liposuction.

Let’s start with perhaps the most difficult terms to understand: forehead/orbital reduction and eyebrow lift via coronal incision.  This is maxillofacial surgery – bone surgery on the face, in other words.  You will need this if you have a prominent, male-type brow ridge with low eyebrows and deep-set eyes, which need to be made more feminine in appearance.

To achieve the forehead/orbital reduction, the surgeons make an incision across the crown of the head (a coronal incision), and the scalp is pulled back to reveal the brow bone, top of the eye socket bones, and top of the nasal bone.  The brow bone and orbital bones are then worked on by both of Facial Team’s maxillofacial surgeons – Dr. Daniel Simon and Dr Luis Capitán.

The relevant section of the frontal brow bone is removed, and the surgeon takes this away to sculpt it.  It is important not to thin this section of bone too much.

You are in safe hands with the Facial Team, as they have done this procedure many times.

While one maxillofacial surgeon works on the section of bone which has been removed, the other surgeon works on the area of bone around and in front of the sinus canals where the removed section will be put back.  The surgeon working on this frontal part of the skull also sculpts the orbital bones as necessary.   The section that has been removed is then replaced and attached with titanium screws. These are smoothed and sculpted down so they don’t show!  Any sculpting to the top of the nose bone is also done at this stage.

My treatment also included rhinoplasty and submandibular liposuction. Rhinoplasty is often referred to as a ‘nose-job’ – it can make the nose a different shape, reduce its size, change the tilt of it, or whatever.  Liposuction is the removal of fat by suction.  A couple of small flexible tubes (cannula) are put into the area to be treated and the fat is sucked out.  You can have it on any part of the body where there is too much fat.   Submandibular liposuction is used to reduce or get rid of a double chin. Both these treatments were performed by Dr. Kai Kaye, who is a very skilful and experienced plastic surgeon.

So to summarize, I had three specialist surgeons working on me at different times during my surgical FFS treatment – two maxillofacial surgeons (Dr. Simon and Dr. Capitán); and one plastic surgeon (Dr. Kaye).  Talk about feeling special!

In order to prevent swelling, I wore a cooling mask on my forehead, which was attached to a sort of portable chiller unit.

I wore this until after lunchtime on the following day, when it was removed, and amazingly – I was ready to go back to the hotel!

Dr Luis Capitán and Dr Daniel Simon, FACIAL TEAM surgeons and specialists in Facial Feminisation Surgery, have employed hiloterapia (controlled cooling therapy) in hundreds of patients since early 2008, proving its many benefits and pioneering the application in the field of Facial Feminisation.

In many patients, surgery brings up feelings of fear, especially about the pain in the postoperative period. We are often asked about the postoperative symptoms frequently associated with surgery, such as pain, swelling and bruising. Traditionally, cooling is recommended after an intervention, often in the form of short-term chemical packs, bags of frozen peas or simply ice to reduce these effects.

However, these are not the best alternatives because the temperature of frozen material is not optimal for soft tissues due to the negative impact of freezing temperatures on the blood supply and lymphatic drainage.

A constant cool temperature around 15 degrees Celsius is sufficient to impede bleeding, swelling and bruising in the vascular territory involved (therefore diminishing pain) yet permit lymphatic drainage, so that the mechanisms for healing may continue.

Hilotherapy is applied by placing one or more insulated devices, anatomically designed to accurately regulate the temperature of the skin and underlying tissues of the particular area. The system works with various models of “masks” connected to a closed-circuit of fluid maintained at a constant cool temperature, which permits the reduction of postoperative inflammation, ergo limiting pain and bruising. As of 2012, we now have access to new mask designs with improved anatomical fit and excellent performance.

The mask, which is relatively comfortable and easy to put on and remove when necessary, allows us to maintain constant optimum temperatures in the affected area, without the risks of freezing of the tissues.

Our patients often comment on the welcome feeling of relief obtained with the use of hilotherapy. In addition, a lower incidence of problems associated with postoperative inflammation as well as a lower demand for analgesia after surgery have been reported.

Many candidates for FFS not only do not require a hairline lowering but even look better with a greater distance between the nasal root and the beginning of the hairline (Alexandra Hamer, Thesis on Facial Gender).

In selected candidates for jaw surgery who require bone sculpture of the mandibular body and angle, we may request a computerized tomography (CT-scan) with special characteristics. Once completed, the images are sent to a partner laboratory that creates an exact three-dimensional model of the patient´s jaw within one week.

 With this 3-D model we can get an accurate idea of the changes needed to soften and redefine the jaw. Moreover we can conduct a virtual surgery on the model, allowing us to know in advance where to focus our work and enabling a more accurate and predictable result.
